Billing

I am an out-of-network provider for all insurance companies. Payment can be made by credit card/HSA/FSA card payments (subject to fee) via Square, check, or cash. 

A SuperBill can be provided upon request, on a case by case basis, with a valid diagnosis code for you to submit. Reimbursement is not always guaranteed as every provider is different. Move to Thrive does not contact insurance providers for SuperBills, so please contact your provider to learn more prior to scheduling services.

Rates for services are subject to change & will be confirmed at the time of scheduling.

The rate is dependent on the type of service provided (e.g. evaluation, play-based OT session, parent coaching, virtual OT, in-service presentation).
